English Education Evaluation Form

Find instructions on submitting and checking the status of your English Education Evaluation eForm.

Declare an English major or change your emphasis

Tips to get started

Prepare a what-if degree audit with your desired major and minor. If you need help generating a what-if degree audit, please contact the LTS Help Desk at 715-836-5711.
Get an advising packet for your proposed English major/minor and mark off courses that you have already taken. If you have completed three or more semesters, sketch out your remaining semesters on the degree plan (the last page of the advising packet). Advising packets are available from the kiosk outside the English Department Office, on the 4th floor of Centennial Hall or you may use the digital copies linked above.
Meet with your ARCC advisor. Bring a copy (either paper or electronic) of your what-if degree audit and your marked-up advising guide.

Please email the English Department (engl.contact@uwec.edu) or stop in the English Department office (Centennial Hall 4102) to make an appointment with the Director of Undergraduate Studies for more advising assistance.

English Ed-Evaluation for Admission to College of Health and Human Sciences

Faculty/staff can access the form, but only students can initiate it. This form is for English Education students to collect throughout their time at UWEC before applying for admission to the College of Health and Human Sciences. Please ask your professor to complete the form (formerly known as "blue sheets") at the end of the semester that you have the course.

Most students will collect a minimum of four evaluations from the following classes:

ENGL 210: Introduction to Critical Studies
ENGL 221: Introductory English Linguistics OR ENGL 284: Intro to Theory & Criticism
Any 200-level English class (other than those listed above)
Any 300-level English class

Log in to the eForm system at https://eform1.uwec.edu. Use your campus username and password to log in.
Click on the "Start New Form" tab at the top of the page.
Select "English Dept" in the dropdown menu.
Select "English Education - Evaluation for Admission to College of Education" from the menu and fill out the form.

Checking the status of your English Ed. Evaluation form

Log in to the eForm system at https://eform1.uwec.edu. Use your campus username and password to log in.
Click on the "Searches" tab at the top of the page.
Select "English Education - Evaluation for admission STUDENTS - F - KV" in the pop-up window. This will show you the status of the forms you have initiated.

English Apprenticeship Form

Faculty/staff can access the form, but only students can initiate it. This form is used for students who are apprenticing/mentoring in an ENGL or WRIT course. Please work with your mentors to complete this form.

Log in to the eForm system at https://eform1.uwec.edu. Use your campus username and password to log in.
Click on the "Start New Form" tab at the top of the page.
Select "English Dept" in the dropdown menu.
Select "Academic Apprenticeship in English" from the menu and fill out the form.

English Departmental Honors Form

Faculty/staff can access the form, but only students can initiate it. This form is initiated by students when they are ready to apply for their first experience for the English Departmental Honors Program. After a student has initiated the form, it may be accessed again when the student completes the first departmental honors experience, and when the student initiates (and later completes) the second departmental honors experience.

Log in to the eForm system at https://eform1.uwec.edu. Use your campus username and password to log in.
Click on the "Start New Form" tab at the top of the page.
Select "English Dept" in the dropdown menu.
Select "Departmental Honors in English" from the menu and fill out the form.
