Philosophy & Religious Studies e-Forms

Your guide to Philosophy & Religious Studies department eForms

Electronic forms, or eForms, are the digital equivalent of the paper forms required for things like registration changes, and applying for apprenticeships and internships. With eForms, signatures can be obtained and the status of forms can be monitored without having to keep track of a physical form. The list below gives some details about e-Forms for Philosophy & Religious Studies students and other common forms, and where to find them.

Only faculty/staff can initiate this form. This form is used for students recruited and closely supervised by an instructor, engaging in a research project with the faculty member.

  • Log in to the eForm system at https://eform1.uwec.edu. Use your campus username and password to log in.
  • Click on the "Start New Form" tab at the top of the page.
  • Select "Philosophy and Religious Studies Dept" in the dropdown menu.
  • Select "PHIL/RELS Apprenticeship 396" from the menu and fill out the form.

Faculty/staff can access the form, but only students can initiate it. This form is used for students who are apprenticing/mentoring in a Philosophy or Religious Studies course. In this course students assist teaching and learning in a course within the department while working closely with a faculty member. Please work with your mentors to complete this form.

  • Log in to the eForm system at https://eform1.uwec.edu. Use your campus username and password to log in.
  • Click on the "Start New Form" tab at the top of the page.
  • Select "Philosophy and Religious Studies Dept" in the dropdown menu.
  • Select "PHIL397/REL397 Student Academic Apprenticeship" from the menu and fill out the form.

Faculty/staff can access the form, but only students can initiate it. This form is used for students who are working with an internship supervisor or faculty member and engaging in professional or occupational projects in the field. Please work with your internship supervisor to complete this form.

  • Log in to the eForm system at https://eform1.uwec.edu. Use your campus username and password to log in.
  • Click on the "Start New Form" tab at the top of the page.
  • Select "Philosophy and Religious Studies Dept" in the dropdown menu.
  • Select "PHIL/RELS Internship 498" from the menu and fill out the form.

Below are instructions on how to find other commonly used student forms and eForms.

This form must be initiated by the student's advisor (or the Chair, or the Dean's Office). Students do not have access to this form. This form is used to substitute Honors courses, study abroad courses, or transfer courses for particular requirements; this form can also be used to substitute a critical studies course from one category into another category.

  • Log in to the eForm system at https://eform1.uwec.edu. Use your campus username and password to log in.
  • Click on the "Start New Form" tab at the top of the page.
  • Select "Registration-Records" in the dropdown menu.
  • Select "Petition for Partial Substitution or Waiver" from the menu and fill out the form.

This form can only be accessed by students and must be initiated by the student. This form is for students wishing to enroll in an independent study or directed study.

  • Log in to the eForm system at https://eform1.uwec.eduUse your campus username and password to log in.
  • Click on the "Start New Form" tab at the top of the page.
  • Select "Registration-Records" in the dropdown menu.
  • Select "Independent/Directed Study" from the menu and fill out the form.

This form must be initiated by the student. If a student encounters a time conflict (class times overlap) while enrolling, the student can use the time conflict e-form to get both instructors' approval to leave early/arrive late for their courses.

  • Log in to the eForm system at https://eform1.uwec.edu. Use your campus username and password to log in.
  • Click on the "Start New Form" tab at the top of the page.
  • Select "Blugold Central" in the dropdown menu.
  • Select "Class Time Conflict Approval eForm" from the menu and fill out the form.

This form is for students wishing to add a class after the first week of the semester, or drop/withdraw from a class after the second week of the semester. This is still a paper form that can be found on the Blugold Central webpage.
