Student Forms: Biology Class Permission Request

The Class Permission Request is currently being used by the Department of Biology. Additional departments will be implementing this form, and when available, updates to this Knowledge Base Article will be provided.
The purpose of this form is to request permission to enroll in a course, where you have tried to enroll in a course, and have received an error message that indicated the course is closed, department or instructor consent in required, and/or that requisites have not been met.
Please fill out the form completely. The instructor (or the Chair) will review your request, and the Academic Department Associate will complete your permission if granted.
