Incoming students should fill out this questionnaire with information on high school and college level classes previously taken, different academic, professional, and personal interests, and concerns for attending college. This information will provide advisors background information on their new advisees in order to give the best guidance in signing up for classes.

Searching for a list of all eForms?
- Navigate to https://eform.uwec.edu.
- Log in using your university username and password.
- Click the Start New Form tab.
- Select the form owner's department from the drop-down menu.
Resuming a started and saved eForm?
- Navigate to https://eform.uwec.edu.
- Log in using your university username and password.
- Click the My Tasks tab to view any forms that require your input.
NOTE: Must have clicked Save and Close for Later. - Click the View Forms in Progress tab to view a form's progress that you initiated.
