The process of submitting your student
agreement form online
Projects lacking all necessary paperwork will be canceled one year from approval date.

Fill out online agreement form.

We will send you an e-mail within a few days informing you of the status of your project.

If pre-approved, the email will have attached to it a document containing your final approved form.
If not pre-approved, the e-mail will state the reasons. Along
with this, the email will contain a link to your originally submitted form.
There you can make your alterations and resubmit the form.

Once you have a "pre-approved" project, print the document attached to your project approval email. (You may want to print
one for your records as well.) Use this form to collect the
signatures of your faculty/staff mentor, community partner, and--of course--your own.

Deliver your signed form to our office -- Human Sciences & Services 205 -- before you
begin your project.

Complete your project.

Based on the project end-date indicated on your form, we will send project completion and evaluation forms to both your faculty/staff mentor and
community partner. When we receive these signed forms back, your project
completion status will be marked accordingly on the
Blugold system. It is important that you make these persons aware that we will be sending these letters a week or two before the ending date indicated on the approved agreement form. This will make the whole process
run more smoothly.

After we have received the evaluation letter from your supervisor and confirmation letter from your faculty/staff mentor, we will send you an
e-mail indicating that either your full (30 hours) or half (15 hours) service-learning
requirement has been met and updated on your degree audit.
