Ver Hagen, David Matthew Memorial


AMOUNT: One at $6000.00
AVAILABLE: Annually
ELIGIBILITY CRITERIA:
This scholarship is offered by Mr. and Mrs. Jan K. Ver Hagen in memory of their son, David Matthew Ver Hagen. The award is given to a “deserving, capable student with a good social conscience and decent grades” who demonstrates financial need. The student must attend UW- Eau Claire.
APPLICATION PROCEDURE:
Application forms are available in the Eau Claire Memorial High School Student Services Office. Applications should be typed and returned to the Memorial High School Student Services Office. Include the following items to the application cover sheet: A resume that includes curricular, co-curricular and extra- curricular activities; community involvement, employment experience, personal interests and hobbies, and any other pertinent information. Include years of participation, leadership positions held and any honors and/or awards. A short essay discussing your qualifications based on the Ver Hagen family’s criteria set forth in the first paragraph of the application form. Maximum length is one page. Two recommendations from people who can speak to your academic ability and potential and your qualifications for the scholarship. A copy of your transcript with grades and class rank through the first semester of the senior year and test scores. A copy of your student aid report (SAR) and a paragraph explaining your financial circumstances.

SELECTION DEADLINE: Prior to July 1
NOTIFICATION:
The Memorial High School Student Services Office will send the recipient a notification letter. The UW-EC Foundation and Financial Aid Offices should each receive a copy of the award letter.
SPECIAL NOTES:
The recipient should send a thank-you note to Mr. and Mrs. Jan K. Ver Hagen in care of the UW-EC Foundation Office, Schofield Hall 214, UW-Eau Claire.
