Foundation Scholarships

Luther Midelfort Foundation

$5,000 in total awards

This award is available to an outstanding undergraduate or graduate student admitted to the nursing program who would most benefit from the scholarship award. GPA will not be the determining factor when making the award. Preference will be given to students who are Luther Hospital employees. Recipients must live in Eau Claire, Barron, Buffalo, Chippewa, Clark, Dunn, Jackson or Trempealeau Counties. Applicants must have a letter of acceptance from, and be enrolled in, a nursing program. Applicants must provide two letters of recommendation: one from a supervisor, instructor or a school official/teacher.

APPLICATION PROCEDURE:

Application Forms: Students who are admitted to the nursing program at UW-Eau Claire will receive an email in late February/early March when the scholarship application is ready and available. The email will include the path to the online application forms located on the Department of Nursing website – www.uwec.edu/CONHS/students/nursscholarships.htm. Part One is filled online and submitted electronically. Part Two (the essay and activities form) is filled online and printed off, signed and submitted to the Dean’s Assistant Office, Nursing 103, by the deadline. The essay must address the eligibility criteria for each of the scholarships that you are applying for. Only one written essay of no more than 300 words will be accepted.

SELECTION PROCEDURE:

The College of Nursing and Health Sciences will screen the applicants based upon the criteria above.

SELECTION DEADLINE:

The Luther Midelfort Foundation will be notified of the recipients by the Dean of Nursing and Health Sciences by May 10.

NOTIFICATION:

The Dean of Nursing and Health Sciences will send a notification letter to the recipients by May 15, with a copy being sent to the Luther Midelfort Foundation. The Dean will complete and submit a “Scholarship Award Form” to the Financial Aid office.

SPECIAL NOTES:

The recipients are required to send a thank-you note within 30 days of receiving notification of the award to the Luther Midelfort Foundation, in care of the UW-Eau Claire Foundation Office, S-214.
