Foundation Scholarships

Luther Midelfort Foundation

This award is available to an outstanding student who would most benefit from the scholarship award and who has a letter of acceptance and is enrolled in a nursing program. GPA will not be the determining factor when making the award. He or she may be a RN, LPN or NA working towards a nursing degree. Students who are Luther Hospital employees and enrolled in the CNP program or distance learning program will be considered. Recipients must live in Eau Claire, Barron, Buffalo, Chippewa, Clark, Dunn, Jackson or Trempealeau Counties. Applicants must have a letter of acceptance from, and be enrolled in, a nursing program. Applicants must provide two letters of recommendation: one from a supervisor, instructor or a school official/teacher.

Application Forms - Students who are admitted to the nursing program at UW-Eau Claire will receive an email in February when the scholarship application is ready and available. The email will include the path to the online application forms – (http://www.uwec.edu/nurs/ on the bottom of the page under HOT LINKS). The forms are filled out and submitted on line, AND a hard copy must be printed off and submitted along with a written essay to the Dean’s Assistant Office, Nursing 277, by the deadline. Written essay - each application must include one written essay in not more than 200-300 words stating why you believe you should be awarded each of the scholarships for which you are applying. The essay must address the eligibility criteria for each of the scholarships that you are applying for. The criteria are listed on the scholarship website http://www.uwec.edu/finaid/aid/FDNscholarships.htm. Financial Need – For scholarships that indicate “financial need required” a FAFSA Financial Aid form must be filled out and there must be some level of financial need. Contact the Financial Aids office for the form at 715-836-3373. Please apply for financial aid by March 1. {APPLICATION DEADLINE:] March 3.

The College of Nursing and Health Sciences will screen the applicants based upon the criteria above.

The Friends of Luther Hospital will be notified of the recipients by the Dean of Nursing and Health Sciences by May 10.

The Dean of Nursing and Health Sciences will send a notification letter to the recipients by May 15, with a copy being sent to the Luther Midelfort Foundation. The Dean will complete and submit a “Scholarship Award Form” to the Financial Aid office.

The recipients are required to send a thank-you note within 30 days of receiving notification of the award to the Luther Midelfort Foundation, P.O. Box 1510, Eau Claire, WI 54702-1510. The Luther Midelfort Foundation will mail the check to the UW-Eau Claire Foundation in June annually for payment to the student at the beginning of the fall semester.
