AMOUNT: 3 @ $500 AVAILABLE: Annually ELIGIBILITY CRITERIA: Candidates must be of junior or senior standing and accepted for an internship in Environmental and Public Health. Candidates must have exhibited a strong academic standard of scholarship and performance. Financial need is to be considered but is not necessarily a basis for selection.APPLICATION PROCEDURE: An email will be sent to all declared majors in the ENPH program in late February/early March when the scholarship application form is available online. The email will include the path to the online application form. The form must be filled out and submitted online AND a hard copy must be printed off and submitted to the Dean’s Assistant office, Nursing 101 by the deadline. Transcripts are required. For scholarships that indicate “financial need required” a FAFSA Financial Aid form must be filled out and there must be some level of financial need. Complete the form on the FAFSA website: www.fafsa.ed.gov/#. If you have questions, please contact the UW-Eau Claire Financial Aid Office at 715-836-3373. Please apply for financial aid by completing the FAFSA form by March 1.APPLICATION DEADLINE: Second Monday in March SELECTION PROCEDURE: Scholarship committees shall be convened by the Department Chair. The funds shall be available for those students who are entering internships or practicum. Recipients shall be selected from the ENPH undergraduate degree program on a revolving basis. The committees shall be made up of the tenured faculty of the Department.SELECTION DEADLINE: Determined by ENPHNOTIFICATION: Notification shall be made by the department chair of ENPH. The chairs will complete and submit a “Scholarship Award Form” to the Financial Aid office.SPECIAL NOTES: Only income generated from the investment of the principal fund shall be awarded annually. Recipients should send a thank-you to Mary Dale, John Gerberich, Jr. and Anne M. Waddington, the children of John and Mary Gerberich, in care of the UW-EC Foundation. |
