ELIGIBILITY CRITERIA:
The recipient must be a Native American student enrolled in the Nursing program at UW-Eau Claire, who demonstrates potential for success in nursing coursework. Both full-time and part-time students are eligible. Applicant must demonstrate the qualities of professional commitment and personal integrity. A student may apply annually, but preference will be given to students who have not previously received the scholarship and to those students nearing the completion of the BSN degree.
APPLICATION PROCEDURE:
Application Forms: Students who are admitted to the nursing program at UW-Eau Claire will receive an email in late February/early March when the scholarship application is ready and available. The email will include the path to the online application forms. PART ONE is filled online and submitted electronically. PART TWO (the essay and activities form) is filled out online and printed off, signed and submitted to the Dean’s Assistant Office, Nursing 103, by the deadline. Only one essay can be submitted. The essay must address the eligibility criteria for each of the scholarships that they wish to apply for.

NOTIFICATION:
The recipient(s) of the scholarship will receive written notification from the Dean of Nursing and Health Sciences with a copy of the award letter sent to the Foundation office and US Bank. The Dean will also complete and submit a “Scholarship Award Form” for each recipient to the Financial Aid office. This form should be sent by June 1.
SPECIAL NOTES:
The recipient is requested to send a thank you letter to US Bank in care of the UW-Eau Claire Foundation Office, S-214.
