AMOUNT: Two awards at $4,000 each, $8,000 totalAvailable: AvailableELIGIBILITY CRITERIA: There are two scholarships available. The first scholarship is available to an undergraduate student and the second scholarship is available to a graduate student. Recipients must be enrolled in the nursing program at UW- Eau Claire, have a minimum GPA of 3.2 and be enrolled for the full academic year in which he/she is receiving the award. $1,000 will be awarded for fall semester and recipient must be in good academic standing (as defined by the university) to receive the spring award of $1,000. The scholarship may be renewed for a second year to the same student provided the recipient write a letter (and submit by the application deadline) to the nursing scholarship committee requesting the scholarship and demonstrating continued success in the UW-Eau Claire nursing program. It is the donor’s wish that this scholarship be used to increase the diversity of men and women in nursing and since males are underrepresented in the nursing profession, male nursing students should be given a preference.APPLICATION PROCEDURE: Application Forms: Students who are admitted to the nursing program at UW-Eau Claire will receive an email in late February/early March when the scholarship application is ready and available. The email will include the path to the online application forms. PART ONE is filled online and submitted electronically. PART TWO (the essay and activities form) is filled out online and printed off, signed and submitted to the Dean’s Assistant Office, Nursing 103, by the deadline. Only one essay can be submitted. The essay must address the eligibility criteria for each of the scholarships that they wish to apply for.APPLICATION DEADLINE: Second Monday in March SELECTION PROCEDURE: The Nursing Scholarship and Awards Committee will review applications and recommend a recipient to the Dean.SELECTION DEADLINE: On or about May 1 NOTIFICATION: The Dean will notify the recipient by letter and submit a “Scholarship Award Form” to the Financial Aid Office.SPECIAL NOTES: Gerald D. “Jay” Woldt, from Minneapolis, established this scholarship in 2010. Mr. Woldt, a native of Chippewa Falls and graduate of Chippewa Falls High School, entered nursing in 1962 when males were just beginning to join the profession. He was a groundbreaker who continued in nursing and obtained his B.S.N. degree and later his M.N. in Nursing Administration with a minor in Education. He became a Certified Registered Nurse Anesthetist (CRNA) in 1970. He taught in the clinical area of Nurse Anesthesia after returning from service in Viet Nam until he retired from the military in 1987. He continued to practice anesthesia in the civilian sector until his civilian retirement in 2006. Because the nursing profession has been so rewarding to him, he would like to encourage more men to enter the profession. His wife also was a CRNA, and his daughter is an operating room nurse in Portland, ME. Recipient must write a letter of thanks to Mr. Gerald Woldt and send it to the UW-Eau Claire Foundation Office, S-214. |
