ELIGIBILITY CRITERIA:
At the time of application, the student must be a declared pre-practicum undergraduate major in Health Care Administration who has demonstrated exceptional dedication to service in the health care field. To qualify for the award, a minimum cumulative grade point average of 3.0 is required. Students who have had volunteer or work experience within an assisted living setting will be given preference.
APPLICATION PROCEDURE:
Applicants must provide an essay on the student’s reason for applying for the award, goals, and special contributions and activities in the major.
APPLICATION DEADLINE:
Due in March. Please check the Health Care Administration scholarship application.
SELECTION PROCEDURE:
A selection committee will review applications and select a recipient.
SELECTION DEADLINE:
Prior to the CHAASE banquet.
NOTIFICATION:
Notification will be made by the selection committee by letter or email. The selection committee will complete and submit a “Scholarship Award Form” to the Financial Aid office. The recipient’s name will be posted on the departmental bulletin board and included in the department newsletter.
SPECIAL NOTES:
The Wisconsin Assisted Living Association (WALA) is the premier statewide association for Wisconsin’s assisted living industry, with over 600 members representing nearly 11,000 residents. The recipient is expected to write a thank you letter to Mr. Jim Murphy, WALA Executive Director, in care of UW-Eau Claire Foundation, S-214.
