AMOUNT: $750 AVAILABLE: Anually ELIGIBILITY CRITERIA: The Jeff Oliphant Scholarship is the highest department award given to an athletic training education student. It is awarded to an outstanding junior or senior student majoring in athletic training who shows great promise as a certified athletic trainer. The scholarship recipient must be able to demonstrate excellence in academic performance, leadership, and service to the profession. Student must have achieved a GPA of 3.0 or higher and have a minimum of one semester remaining before graduation or be continuing in graduate school at UW-Eau Claire.APPLICATION PROCEDURE: Obtain application form from the Department of Kinesiology. The application form, along with appropriate supporting information, should be returned by March 15 to the Department of Kinesiology Office.APPLICATION DEADLINE: Last Friday in March SELECTION PROCEDURE: The Department of Kinesiology Scholarship Committee will review the applications and in consultation with the Oliphant family select the recipient.SELECTION DEADLINE: Prior to the end of the spring semester.NOTIFICATION: The Chair of the Scholarship Committee or the Chair of the department will notify the recipient. The Department Chair will submit a “Scholarship Award Form” to the Financial Aid Office.SPECIAL NOTES: It is suggested that each recipient send a thank-you letter to Ruth Oliphant in care of the Foundation Office, S-214. This scholarship is given in memory of Jeff Oliphant who died suddenly on August 12, 2005. Jeff was a beloved member of the UW-Eau Claire faculty and pioneered the accredited athletic training education major at UW-Eau Claire. This scholarship honors his life’s work and dedication to athletic training excellence. The scholarship is given to a promising athletic training education student who will uphold the standards of professionalism and excellence by which Jeff lived. |
