Foundation Scholarships

Luther Midelfort Foundation - NP

Two at $2,500, $5,000 total

This award is available to an outstanding student(s) who would most benefit from the scholarship award and who is/are currently enrolled in the UW-Eau Claire graduate nursing program, Nurse Practitioner (NP) role preparation. The applicant must also be within one year of completing their program of study and registered for at least 7 credits or more during the final two semesters. Students who are Luther Hospital employees or considering becoming employees of Luther may receive special consideration. Financial need also will be a consideration in the selection. Recipients must be within a year of completing their degree program. Applicants must provide at least one letter of recommendation from a supervisor, instructor or a school official/teacher.

Application Forms: Students who are admitted to the nursing program at UW-Eau Claire will receive an email in late February/early March when the scholarship application is ready and available. The email will include the path to the online application forms located on the Department of Nursing website – www.uwec.edu/CONHS/students/nursscholarships.htm. Part One is filled online and submitted electronically. Part Two (the essay and activities form) is filled online and printed off, signed and submitted to the Dean’s Assistant Office, Nursing 103, by the deadline. The essay must address the eligibility criteria for each of the scholarships that you are applying for. Only one written essay of no more than 300 words will be accepted. Financial Need – For scholarships that indicate “financial need required” a FAFSA Financial Aid form must be filled out and there must be some level of financial need. The form is on the FAFSA website: www. fafsa.ed.gov/#. If you have questions regarding the FAFSA form, please contact the UW-Eau Claire Financial Aid Office at 715-836-3373. Please apply for financial aid by March 1.

The College of Nursing and Health Sciences will screen the applicants based upon the criteria above.

The Luther Midelfort Foundation will be notified of the recipients by the Dean of Nursing and Health Sciences by May 10.

The Dean of Nursing and Health Sciences will send a notification letter to the recipients by May 15, with a copy being sent to the Luther Midelfort Foundation. The Dean will complete and submit a “Scholarship Award Form” to the Financial Aid office.

The recipients are required to send a thank you note within 30 days of receiving notification of the award. Thank you notes should be written to the Luther Midelfort Foundation, in care of the UW-Eau Claire Foundation Office, S-214.
