Foundation Scholarships

Luther Midelfort Foundation - CNP

This award is available to an outstanding student who would most benefit from the scholarship award and who is a full-time student enrolled in the UW-Eau Claire MSN program, Nurse Practitioner (NP) role preparation. Students who are Luther Hospital employees or considering becoming employees of Luther may receive special consideration. Financial need also will be a consideration in the selection. Recipients must be within a year of completing their degree program. Applicants must provide two letters of recommendation: one from a supervisor, instructor or a school official/teacher.

Application forms – Students who are currently enrolled in the UW-Eau Claire MSN program - NP role preparation and who are within a year of completing their course of student will receive an email from the COHNS dean’s office. The email will include the path to the online application forms – (http://www.uwec.edu/nurs/ on the bottom of the page under (HOT LINKS). The forms are filled out and submitted on line, AND a hard copy must be printed off and submitted along with a written essay to the Dean’s Assistant Office, Nursing 101, by the deadline. Written essay – each application must include one written essay in not more than 200-300 words stating why you believe you should be awarded each of the scholarships for which you are applying. The essay must address the eligibility criteria for each of the scholarships that you are applying for. The criteria are listed at http://www.uwec.edu/finaid/aid/FDNscholarships.htm. To verify financial need, a FAFSA Financial Aid form must be filled out and there must be some level of financial need. Contact the Financial Aid office for the form at 715-836-3373. Please apply for financial aid by March 1.

The College of Nursing and Health Sciences will screen the applicants based upon the criteria above.

The Friends of Luther Hospital will be notified of the recipients by the Dean of Nursing and Health Sciences by May 10.

The Dean of Nursing and Health Sciences will send a notification letter to the recipients by May 15, with a copy being sent to the Luther Midelfort Foundation. The Dean will complete and submit a “Scholarship Award Form” to the Financial Aid office.

The recipients are required to send a thank you note within 30 days of receiving notification of the award. Thank you notes should be written to the Luther Midelfort Foundation, in care of the UW-Eau Claire Foundation Office, S-214.
