AMOUNT: $500 AVAILABLE: Annually ELIGIBILITY CRITERIA: The student must be a declared Health Care Administration major of junior or senior academic standing, not yet in practicum during the semesters that the scholarship is received. To qualify for the award, a minimum cumulative grade point average of 3.0 is required. Students who have demonstrated leadership within the student organization of ACHCA will be given preference.APPLICATION PROCEDURE: Applicants must provide an essay on the student’s reason for applying for the award, goals, and special contributions and activities in the major.APPLICATION DEADLINE: SELECTION PROCEDURE: A selection committee will review applications and select a recipient.SELECTION DEADLINE: NOTIFICATION: Notification will be made by the selection committee by letter. The selection committee will complete and submit a “Scholarship Award Form” to the Financial Aid office. The recipient’s name will be posted on the departmental bulletin board and included in the department newsletter.SPECIAL NOTES: Recipient should send a thank-you note to Dr. Gene Decker in care of the UW-Eau Claire Foundation. |
