MAILED: Sept. 17, 2002
EAU CLAIRE — The University of Wisconsin-Eau Claire School of Nursing will offer free health evaluation clinics for infants and children up to age 15 on September 30, 2002.
Health evaluations will begin at 4:00 p.m. at the UW-Eau Claire School of Nursing. Clinics will include a physical assessment, developmental screening, nutrition assessment, vision and hearing screening, and a review of the child’s health history.
Nursing graduate and undergraduate students will conduct the clinics under the supervision of certified nurse practitioners. Supervisors will be available to sign physical examination forms for preschool, day care, kindergarten, camp and some sports forms. Referrals will be made as necessary.
To create a positive health care experience, the clinics are paced to allow time for explanation of procedures to the child. Participants should allow two hours for a complete screening assessment. Follow-up home visits may be necessary in order to provide a summary of clinic findings.
Appointments are limited and will be taken on a first-come, first-served basis. For more information or to make an appointment, call (715) 836-5515.
