Registration Form

Please complete and submit the following Registration Form by January 27, 2012. Then complete the registration process by mailing the entry fee of $30.00 per team, payable to UW-Eau Claire to: Math Meet, Department of Mathematics, University of Wisconsin-Eau Claire, Eau Claire, WI 54702-4004. (Registration froms should be postmarked no later than Friday, February 3, 2012 to be received by the 6th.) Fees are used to cover the cost of contest materials, awards, and refreshments.

  Class AAA schools are those with 600 or more students enrolled in grades 10-12.
    Class AA schools are those with enrollments between 200 and 599, inclusive.
    Class A schools are those with enrollments less than 200

Remember, each team must have a different sponsor as sponsors will be "swapped" to proctor other teams during the events. There will be a Proctor Meeting no later than 9:40 a.m. on the day of the Meet to answer questions and distribute instructions.

Provide the Team name and Team members' names as they would like them to appear on their certificate of participation. List the members in order of grades 12, 11, 10, 9. Complete the T-shirt size requirements. T-shirts for each team member and the team's sponsor are complementary.

10. If we need to cancel due to inclement weather, we will contact a person between 6:00 p.m. Friday, February 17 and 6:00 a.m. on Saturday, February 18, 2012. Please list the name of the person to contact and the phone number to call.
