To participate in this conference, discuss your attendance with your campus LTDC Representative or learning technology office. Once you have confirmed your participation, please fill out this form and submit it no later than September 21, 2009. All participants and presenters must submit this registration form.
Each conference participant is responsible for making their own hotel reservation
at the Double Tree Hotel, 525 W. Johnson Street, Madison. To assure room availability at the conference rate, reservations must be booked before Sept 29.
A block of rooms is being held under the code “UWS LTDC.” Participants will need to use their own credit cards to reserve rooms.
Conference registration, hotel stay (up to two nights), and all meals at the conference [Oct 28 – Breakfast, Lunch, Dinner, Oct 29 – Breakfast & Lunch] will be covered. Travel expenses (except those local in Madison) will also be covered as needed. We encourage carpooling to reduce cost and better networking opportunities if possible. Meals incurred during travel will be covered according to State Travel Guidelines.
All travel expense reimbursement will be handled via a TER process to UW System. Please fill out the TER form and forward or mail the completed TER form to Amy Maurer, deadline: Dec 1, 2009.
