Taxpayer Guide to Identity Theft

We know identity theft is a frustrating process for victims. The Internal Revenue Service has put together a Taxpayer Guide to Identity Theft.

What is tax-related identity theft?

Tax-related identity theft occurs when someone uses your stolen Social Security number to file a tax return claiming a fraudulent refund.

Generally, an identity thief will use your SSN to file a false return early in the year. You may be unaware you are a victim until you try to file your taxes and learn one already has been filed using your SSN. Read more.....

 

 

Voluntary Separation Incentive Program

On March 12, 2015 the Chancellor announced the implementation of a voluntary separation incentive program that will be available to eligible employees in select departments.

Employees have been notified via email and U.S. Postal Service on March 16, 2015 that they are eligible to apply for consideration to participate in the Program. We have the following information for your reference at this time:

BENEFIT INFORMATION RELATED TO POSSIBLE BUDGET IMPACTS

Because of the proposed budget situation and its impact on UW-Eau Claire, there may be a need to non-renew or lay off some employees.  In the event this becomes necessary, this page is intended to offer some explanations and to provide information on the impact on employee benefits.

 

 

Anthem Blue Cross Information - Update

Update: Health insurer Anthem Blue Cross Blue Shield has announced that members (current and former) dating back to 2004 are being offered identity repair assistance and credit monitoring services via AnthemFacts.com. Affected members can start accessing these services prior to receiving a mailed notification from Anthem and can enroll any time during the 24-month coverage period. Learn how to sign up at AnthemFacts.com or call toll free 1-877-263-7995 from 9:00 a.m. to 9:00 p.m., Monday through Friday, Eastern Time.

Last week Anthem Blue Cross Blue Shield Health Plan announced that it was successfully breached by a sophisticated, external cyber-attack.  No computer systems related to UW were targeted in this breech. This breech may impact current and former members of our community who have health coverage and dental coverage through the following plans currently or previously offered by the State of Wisconsin Group Health Insurance Program:

  • Anthem Blue Preferred-Southeast Network
  • Anthem Blue Preferred-Northeast Network
  • Anthem Blue Preferred-Northwest Network
  • Anthem Dental Blue
  • Standard Plan participants from 1994-2005 (Blue Cross Blue Shield administered the Standard Plan from 1994-2005)

This event does not affect members of the other health plans offered through the group health insurance program.

The type of information that the attackers accessed includes member names, birth dates, member health ID numbers, Social Security numbers, mailing and email addresses, and employment information. Thus far, Anthem officials believe that specific medical information and credit card numbers were neither targeted nor compromised.  .

Anthem will contact current and former members whose information has been accessed with detailed information and instructions. In the meantime, more information is available at www.AnthemFacts.com or by calling Anthem toll free at 1-877-263-7995.  We encourage you to monitor your credit ratings and contact Anthem with questions or concerns.

We’ve recently received additional information from UW System regarding this event.

Con artists have seized on last week's massive hack of Anthem, the second largest health insurance company in the US, as an opportunity to perpetrate phishing scams. Several reputable sources have confirmed that past and present Anthem customers--services like Blue Cross Blue Shield--have received phishing emails that purport to be from the company.  Read more.

Home Address Information

Student employees, Classified, LTE and Unclassified should login to the "My UW Portal" to verify your home address. Your home address is the address that your W2 will be mailed to.

Staff, your home address is only maintained in HRS. Human Resources does not maintain addresses for staff in CampS.

Students, you must maintain your home address in both systems, CampS and in HRS.

Health Insurance Marketplace

Beginning in 2014, the federal Affordable Care Act (ACA) requires almost everyone to obtain health insurance for themselves and their dependents or pay a penalty when filing their tax returns.  The Marketplace (or Exchange) is a new option for people to obtain health insurance.

For employees that are currently under a State Health Plan and depending on your circumstances, the “Marketplace” options may be of interest to you.

Leave Reporting for 9-month Academic Year Contracts

If you are a 9-month (c-basis) contracted employee ELIGIBLE FOR SICK LEAVE, this means you will be entering leave reports from September through May. If you are unsure, please contact human resources.

FAQ’S:

Q: What if I don’t use any sick leave, do I still have to enter a leave report?
A: Yes, you still need to enter a leave report. You will click on “enter absence” and then the absence name will be, “No leave taken”. By reporting no leave taken, you receive your sick leave balance for that month.

Q: What date should I use for no leave taken?
A: You can use the first of the month you are reporting for. For example, if you did not take any leave during the dates of the September time frame you would put the start date as “9/1/2014”, select, “no leave taken” and then put “9/1/2014” in the end date. 

Q: What date(s) should I use if I have sick leave in a month?
A: Use the exact date you were gone. You can enter a range if you are gone the exact amount of hours for consecutive days Monday-Friday. If you are gone on a Friday and gone on the following Monday, you would need to enter two separate leave reports.

Q: What if I am entering a half day (4 hours)?
A: Go to entry type, select the drop down “hours per day”. In the hours per day box, you will enter “4”. Click “Calculate hours” and “4” hours should pop up in the duration box.

Q: There is an original start date box, do I have to put in a date?
A: No, you can leave it blank.
