2009 "It's Your Choice"
|
The 2009 Dual Choice enrollment period will be held from
Monday, October 5 through Friday, October 23, 2009. During this
time, currently insured employees may switch from one health plan to
another or from single to family coverage without any waiting period for
pre-existing conditions. Changes include (but are not limited to):

OptumHealth Vision will no longer be
available after December 31, 2009; a new vision plan will be offered.
·
Open enrollment - DentalBlue
·
No open enrollment for Epic Dental and Excess
Medical Insurance
Information will be sent to eligible employees as it becomes
available.
NOTE: In early October, "It's
Your Choice" booklets will be e-mailed to
eligible employees. E-mail messages will include a link to the Department of
Employee Trust Fund's (ETF) Web site to obtain the "It's Your
Choice" booklet. Note: Paper copies of the booklet will
be available at the Benefits Fair for those who need them.  |

Presentations Set for September – November 2009 |
The Department of Employee Trust
Funds (ETF) will offer free public presentations on Wisconsin Retirement
System (WRS) benefits in September, October, and November. These WRS
workshops are valuable opportunities for WRS members, spouses, and other
individuals seeking general information on retirement issues and
ETF-administered benefit programs. Those considering or planning retirement
in the near future are encouraged to attend. Check

Personal Information

"My Human Resources" is where you can verify your personal information along with
your previous UW-Eau Claire employment information and make address
changes. "My Human Resources" is available through your
browser anytime throughout the year. Where can I find it? From HR's
Web site, click on "My Human Resources." You will be asked
to enter your campus user name and password. You may review employee
information and employment history.
If there are changes that need to be made to your data, e-mail
HR Data Review, persnl02@uwec.edu.

Personal Changes
If your family status is changing, please check with Human
Resources to see if there are insurance coverage changes. Home
address and emergency contact change updates can be made by logging into
"My Human Resources" from UWEC Human Resources homepage at http://www.uwec.edu/hr. Family
status changes may include:
marriage or divorce
-
birth or adoption of a child
-
a child reaching 19 years
old and not a student
-
Effective January 1, 2010, coverage
will extend through the end of the month in which the child turns 27.
Student status is not required.
Campus Directory Changes
According to Wisconsin public records laws, an employer cannot
publish or release an employee's home address and telephone number
electronically or in the UW-Eau Claire campus directory unless authorized by the employee. All employees
will soon be receiving an e-mail verifying whether or not their information
will be published in UW-Eau Claire's campus printed directory and the
online directory. You may check this information anytime by logging
into "My Human Resources" from our Homepage. |

Criminal Background Checks
|
When hiring new employees, please keep in mind that a criminal
background check must be performed on candidates before a job is offered.
To find out if a prospective employee already has a background check on
file, or if you have questions or concerns, contact Melissa Binick in HR at
36-2515 or binickms.

Worker's Compensation
|
We are taking this time to remind all
employees and supervisors that any work-related injury or
illness must be reported immediately, and the correct forms must be
completed by the employee and supervisor.
The Department
of Workforce Development regulations (DWD 80.02 (3)(b)) indicate that
Worker's Comp claims be submitted to the Worker’s Compensation Coordinator
in the Human Resources office immediately following an injury. Untimely submissions are subject
to fines. The forms can be found on the HR Web site under

2009 Charitable Campaign
|
In late September all faculty and staff
will receive the United Way packet, which will include a
pledge
card and brochures. State Employees Combined Campaign will be sent
out separately via email with online pledge cards. Direct questions

or e-mail bowersba.
Several UW-Eau Claire employees took
part in the Annual Day of Caring on August 21, combining a day of
furlough with community support. Thank You to
all who participated!

Staff News |
Reminder To Supervisors of Classified Staff |
Fiscal year 2008-09 performance evaluations were due in Human
Resources on July 1, 2009. Annual performance evaluations are
required for all Classified Staff. The performance evaluation form
may be accessed online at: http://www.uwec.edu/hr/FormsAlpha.htm, under “Performance Evaluation.” If you have not
submitted your evaluations yet, please do so as soon as possible and submit
the original signed form to Human Resources. Contact Amy Nelson with questions and concerns
regarding performance evaluations. |

Timesheets Online
|
Classified , WPEC , and LTE timesheets, along with pay
schedules , can be found on Human
Resources' Web site.  |
NOTE: When
using online timesheets, please access them from HR's Web site through Internet Explorer (not Excel) so you are using the most current pay period. If
you use a "saved" timesheet from your computer, the dates may be
incorrect. Timesheets are due in the Human Resources office,
Schofield 219, by noon on the second Thursday of the pay period. Be
sure to print on the appropriate color paper.

Sabbatical and Cash Payment Option
|
Notices for the classified sabbatical and
cash payment options will be sent to eligible employees in November.
Eligibility criteria varies between union contracts. Consult your
union contract if you have questions about this benefit.  |

Leave Statements |
Academic
year unclassified employees will receive the first monthly leave statement
for the academic year on or about 10/01/09. Please remember to
complete it and send it to your supervisor each month within five days of
receipt. Those employees with a 2009 summer session appointment
should receive their summer session leave statement in the first part of
September. Because of furlough reporting, some of you will receive an extra
page on your leave report or if you do not accrue leave, you will receive
just furlough report to report your furlough usage. If you are
missing a previous report, you may access a generic statement on Human Resources' Web site in the forms section. Refer questions to Barb Bowers at 36-3871 or e-mail bowersba.

Unclassified
Leave Reports |
A
leave report must be submitted each month, regardless of whether any leave
was taken. Failure to submit leave reports on a timely basis will
result in a reduction to your sick leave accrual.
Reduction
of Sick Leave Accrual for Unclassified Staff Failing to Report leave
Usage. Effective with sick leave accrued in fiscal year 2008-2009, any
employee who fails to file a report on leave usage as required by UW System
policy in one or more months of any year shall not be permitted to accrue
sick leave for that year in an amount exceeding the cap established by
s.40.05(4)(bp)1., Wis. Stats. (i.e., 8.5 days for an annual appointee or
6.4 days for an academic year appointee). Reduction will occur

Medical Certificate
Written
certification is necessary from a health care provider of the medical
necessity for use of sick leave used for more than five consecutive full
work days, except when the use of sick leave is authorized in advance,
pursuant to the Wisconsin or Federal Family and Medical Leave Act.

Student Payroll News |
Student Payroll
Deadlines |
Timesheets are due in the Human Resources
office (Schofield 219) by noon every other Friday (unless otherwise
noted). Mandatory furlough will result in early submission for some
pay periods. Please refer to the Student Payroll Calendar on HR's Web
site at http://www.uwec.edu/hr/schedules.htm.
Student employee payroll forms are due in the Human Resources office the
Monday prior to submitting a timesheet. These forms include direct
deposit, I-9, and W-4. All forms may
be found on HR's Web site by
clicking on forms and scrolling down to student forms. Please make
sure all forms are filled out completely before submitting to student
payroll.
A delay in submitting any of the above
mentioned payroll forms may cause a delay in the student being paid. Refer
questions to Julie Carr at 36-4068 or carrjk.  |

Eligibility |
The
academic year eligibility session begins August 23, 2009 and ends May 15,
2010. Please make sure ALL student workers have a new academic year
eligibility form on file before submitting their first academic year
timesheet. Please refer to the Human Resources' Web site http://www.uwec.edu/hr/student.HTM for the payroll schedule.

Selective Service Compliance Policy

International Students
|
All newly-hired international student
employees need to make an appointment to see Cheryl Hauser (e-mail hauserc) to complete their payroll
forms (i.e. I-9, tax forms including tax treaty forms). Supervisors
should enter the student's online eligibility form as soon as they want
to start the employment process. International students must not
begin working until they have received approval from the Center for
International Education to work on campus and completed forms with
Cheryl.  |
