Fall Enrollment Opportunities
From October 5, 2009 through November 13, 2009
For the employee-pay-all optional benefit plans

Anthem DentalBlue Dental Insurance

Eligible employees may enroll for coverage, change plans within Anthem DentalBlue (e.g., change from the HMO to the Supplemental plan) and add eligible dependents during this enrollment period. 

A domestic partner (and his/her dependents) is eligible for coverage under this plan.  Unmarried children are now eligible for coverage as your dependent until the end of the month in which he/she turns age 27.

IMPORTANT:  Once enrolled in Anthem DentalBlue you must
remain covered for the entire calendar year.

Preferred PPO Benefit Change
Effective January 1, 2010 dental implants will be covered under the Preferred PPO plan only.  Benefits are payable in accordance with schedule of benefits up to the annual maximum of $1,000 for all eligible dental expenses.

NOTE:  If the Supplemental Plan is selected, the employee must carry basic dental benefits under a health plan within the State Group Health Insurance program.  The employee’s eligible dependents do NOT need to be covered under the State Group Health Insurance program in order to be eligible for the Supplemental Plan.

VSP Vision Insurance (replaces OptumHealth)


OptumHealth Vision Insurance will not be available after December 31, 2009.  Instead, VSP vision insurance will be offered. Eligible employees may enroll in VSP from October 5 – November 13, 2009 for coverage effective on January 1, 2010.  If you are currently enrolled in OptumHealth Vision and you would like to maintain vision insurance, you must enroll in VSP during the open enrollment period. 

A domestic partner (and his/her dependents) is eligible for coverage under this plan as are unmarried children until the end of the month in which they turns age 27.

IMPORTANT:  Once enrolled for a calendar year, you must
remain covered for the entire calendar year.

VSP has a different benefit design than OptumHealth so please review the benefits offered in 2010.  VSP covers contact lenses with no copayment and will cover glasses (lenses and frames) every calendar year.  The provider network includes more private providers rather than stores like Walmart, Shopko or Target.

VSP is sending you a packet of information at your home address.  If you haven’t received the packet by October 12, please contact VSP at 1-800-400-4569 to request one.

UW Employees Inc Life Insurance

UW Employees Inc Life Insurance is a term life insurance policy.  Any eligible employee can enroll for coverage during this period, regardless of health.  This policy provides an inexpensive way to increase your overall life insurance coverage.  There are no changes to the premium or the benefit levels for 2010.

Expanded Coverage for Domestic Partners and Dependent Children under Age 27

Effective on January 1, 2010, eligibility for State Group Health, State Group Life Insurance and Epic Dental and Excess Major Medical plans will be expanded to include domestic partners.  Coverage will also be expanded under health and EPIC to include eligible dependent children under age 27. 

If you previously filed a UW System Affidavit of Domestic Partnership (UWS93) to enroll your domestic partner in benefits that currently provide domestic partner coverage and you would like to add a domestic partner to health, State Group Life Insurance and/or EPIC, you will need to submit a notarized Department of Employee Trust Funds (ETF) Affidavit of Domestic Partnership (ET-2371) to ETF before the domestic partner and children may be enrolled.  In order to add a domestic partner to coverage effective January 1, 2010, you must submit a notarized affidavit to ETF on or before December 30, 2009 and submit a copy of the affidavit and appropriate enrollment application to your institution’s benefits office.

Once a domestic partnership is established with ETF, you have 30 days from the domestic partnership effective date to enroll a domestic partner in any benefits that provide domestic partner coverage.  If you would like to enroll a domestic partner in benefits effective January 1, 2010, it is recommended that you submit enrollment application during the fall enrollment period but you must submit applications no later than 30 days after the effective date of your domestic partnership with ETF.

Employees who currently carry domestic partner coverage under Anthem DentalBlue Dental Insurance, Individual and Family Life Insurance or Accidental Death & Dismemberment Life Insurance do not need to take any action to keep the domestic partner on the plan.

Effective January 1, 2010, dependent children are eligible for coverage until the end of the month in which they turn 27 under State Group Health Insurance, EPIC, Anthem DentalBlue Dental Insurance and VSP Vision Insurance.  An unmarried child over age 19 no longer needs to be a full-time student or dependent upon you or the other parent for at least 50% of his/her support. 

Important Tax Implications

If you cover a dependent child, domestic partner or domestic partner’s children on your health insurance who are not your tax dependents under Internal Revenue Code Section 152, the fair market value of insurance coverage provide to your non-tax dependents will be added to your earnings as imputed income.  Your taxes will then be calculated on your regular gross income plus the fair market value of the health insurance.  Additional fair market value information and an imputed income calculator is at this link http://etf.wi.gov/employers/benefit_programs_imputed_income.htm. If you cover a non-tax dependent on any benefit other than health insurance, such as EPIC, VSP or Anthem DentalBlue, the monthly premium will be taken on a post-tax basis.
