
EMPLOYEE REIMBURSEMENT ACCOUNT (ERA) ENROLLMENT

The annual enrollment period for the 2010 plan year is
October 5 – November 13, 2009
The Employee Reimbursement Account (ERA) program allows employees the option to set aside pre-tax income for eligible medical and/or dependent care expenses.
The ERA brochure contains important and useful information. We suggest that you retain the URL for reference throughout the year. The ERA program is administered by Fringe Benefits Management Company (FBMC).

QUICK FACTS ABOUT ERA
- Offers a tax-free Medical Expense Account.
- You can be reimbursed for non-covered medical expenses, such as copayments, eyeglasses, laser eye surgery, in vitro fertilization and medical mileage. A more complete list of covered expenses is found on page 14 of the brochure.
- The minimum annual contribution is $100. The annual maximum contribution is $7,500.
- New for 2010 - The myFBMC CardSM. Participants in the Medical Expense Account will receive an electronic payment card which allows FBMC to electronically deduct funds from your Medical Expense Account and can be used for prescription drug copayments, over-the-counter items and other eligible expenses. You do not have to use the card if you choose not to. You will only need to submit a claim for eyeglasses or cosmetic dental services. - Offers a tax-free Dependent Care Account.
- You can be reimbursed for eligible dependent care expenses for children under age 13 and/or eligible adult or elder care expenses.
- The minimum annual contribution is $100. The annual maximum contribution per family $5,000. See page 18 of the brochure for more information about contribution limit restrictions.
- Expenses for the Medical Expense Account and the Dependent Care Account must be incurred between January 1, 2010 and March 15, 2011 and must be submitted to Fringe Benefits Management Company by April 15, 2011.
