The Qualtrics Survey System is an application that provides the tools you need to create and administer surveys and analyze the data received. This document will help you get started in the creation of a survey.
The Survey Wizard is recommended for someone who has never used Qualtrics before. You will be prompted with a step by step process to create a customized survey.
In the My Surveys tab, on the My Surveys toolbar, click CREATE SURVEY
OR
Select the Create Survey tab
The How do you want to build your survey? page appears.
Click SURVEY WIZARD
The Survey Creation Wizard appears.
In the Survey name text box, type the desired name for your survey
From the Select a user or group account location for this survey pull-down menu, select the desired location for the survey
NOTE: My Account will make the survey yours alone.
From the Survey Look and Feel section, select the desired design (e.g., Midnight Watch)
Click NEXT>>
The Add Questions page appears.
Using the pull-down menus, select the desired question type to add
NOTES:
Options in the pull-down menus will vary based on your initial question type.
A sample question for that question type is displayed.
HINT: To add questions later, click DON'T ADD ANY QUESTIONS

Click ADD THIS QUESTION>>
The Add Questions page refreshes.
In the Enter question text below text box, type your question
EXAMPLE: Type Which building do you spend most of your time in?
Click and type to add additional elements or information appropriate to your question

OPTIONAL: Use the formatting tool bar to format the text as desired
Click NEXT >>
Additional options for your question type appear.
Click NEXT >>
Additional options and a preview of your question appear.
When you are satisfied with the question setup, click FINISH
To add additional questions, repeat steps 7 - 14
When you are done adding questions, click STOP ADDING QUESTIONS
Click FINISHED
NOTES:
A new survey is created.
You are on the Edit Survey tab.
