Information Release

If you were not claimed on your parent(s) tax form*, but would like the Financial Aid Office to talk to your parent (or other designated individual) about the status of your financial aid application, processing procedures, eligibility and award amounts, extenuating circumstances, appeal procedures and the implications of additional resources, you must complete an Information Release Form.

*Please be aware, there are circumstances were you may be asked to sign and return this form, even if your parent(s) claimed you on their taxes.
