Apply to EOC Important information. Please read carefully!

As a federally funded program that reports to the U.S. Department of Education, the EOC is required to collect such information as your Social Security Number, date of birth, and income information, to document client eligibility to receive services. Please complete this application with the most current and accurate information.

Some Internet Browsers will ask if you want to run scripts or ActiveX controls which will look something like this

Simply click on the yellow pop-up and ALLOW BLOCKED CONTENT. Another pop-up screen will ask if you are sure you want to do this. Click YES. You must do this before you fill anything out on the form or you will lose the information you have already filled out.
