Graduate Application Materials

Welcome to the web page created by the Department of Communication Sciences and Disorders to assist you in applying to our graduate program here at UW-Eau Claire. It is our hope that this page can act as a basic reference to guide you through the application process. Each year the Department publishes a pamphlet describing the Graduate Program in Communication Sciences and Disorders at UW-Eau Claire (pdf document). This brochure goes into detail about applying to our program, our graduate degree requirements, and our department in general. The pamphlet was put together specifically to answer all of the questions we could anticipate you might have. There are some very important things you should know about our program which are summarized below.

A Brief Checklist of Application Requirements:

An Overall GPA of 2.75 (or a 2.90 in the second half of undergraduate coursework).
A Communication Sciences and Disorders coursework GPA of 3.0.
A minimum of 30 semester credits in CSD courses before you start your graduate program.
At least a B in each practicum that was graded.
Have your GRE results sent to us.
Submit the CSD application.
Have at least 3 letters of recommendations sent to us.
Submit the University application for admission to graduate study.
Have official transcripts sent from every university/college you have attended.
If you plan to apply for a scholarship or graduate assistantship additional applications and copies of your recommendation letters will be needed.

All of the materials needed to apply to our program are available at this site or through links to other UW-Eau Claire sites. All of the CSD application forms can be word processed directly. You can save each form to your disk and then type in the required information before printing it out. The forms may also be printed and then you can type or write in the information. If you choose to type in the spaces provided, there are a few things to remember. The form cannot be altered, so if information vanishes in a table or lines shift inappropriately, use recognizable abbreviations to fit the information in the space provided. If you are typing in several lines of text, there is no way to get the form to automatically return you to the left as you go to the second line, but you can move the curser to the text box on the next line and keep going. At the end, save the form under a new name on your disk to keep all the information you have typed in. All forms will eventually need to be printed out and sent to us here at the University of Wisconsin-Eau Claire.

The application process here at UW-Eau Claire involves getting the right information to the right offices. In fact, there are three separate offices which manage different parts of the application process: our Department, the Admissions Office, and the Graduate Office. A quick way to keep track of where forms need to be sent is to print out a copy of our CSD Application Checklist (Word document). The checklist outlines who gets what.
