A diploma replacement may be requested by printing the following form and mailing it to the address indicated on the form. The degree will be verified before the replacement is produced. A fee of $40 is charged for each replacement diploma.
Note: The diploma size changed as of Spring '06 commencement, so previous graduates now have the option of replacing the smaller 6" x 8" diploma with the larger 8" x 10" diploma, upon payment of the $40 fee.
