Evaluation of Team Members

On this form you will evaluate the work of the members of your group, other than yourself, by selecting the appropriate responses. This procedure allows me to know who has been contributing to group projects and then adjust the project grade accordingly.
Up to three persons can be evaluated. If you had only two members in your group, for example, response only to one set of questions. You have six response possibilities in the pull-down menus below: consistently, almost always, usually, seldom, never, and not appropriate.
