Before beginning to practice with any team, a student-athlete must have a physical if a new candidate or a health screening if a returning athlete. Because the university does not carry health insurance for its athletes, all candidates must provide proof of insurance through a personal policy or their parents' policy. A varsity candidate must also attend a mandatory meeting at which forms will be completed and rules and regulations discussed.

Procedure:

1. Go to the eligibility form
2. Sign in with your UWEC username and password
   • If you stop working on the form once started, it will time out at 20 minutes but will save your data and you can complete it when you log on again.

MANDATORY MEETING -- At the mandatory meeting for your sport, you will be asked to complete a WIAC Form A and NCAA Student-Athlete statement.  These forms WILL NOT be available online.  You will also be asked to sign a statement indicating that you have read and understand everything you filled out online. - Meeting Schedule

INSURANCE CARD -- Before you can begin practice, you must turn in a copy of both the front and back side of your health insurance card.

NEW TRANSFER STUDENT – Any student-athlete, who has ever attended any other institution (other than during summer school) and has never completed athletic eligibility forms at UWEC previously, must complete appropriate transfer forms at the team organizational meeting.

INTERNATIONAL STUDENT – Any student-athlete who is an international student must see the Athletics Directory (Scott Kilgallon), Compliance Officer (Tim Petermann) or Senior Women’s Administrator (Leslie Huntington) to complete necessary NCAA paperwork.
